The assets and liabilities statement is a bilingual (DE/EN), fillable template in three parts: real estate, other liabilities and other assets. The bank wants it in addition to the self-disclosure, because the self-disclosure describes the current month, income and outgoings, while the statement describes the stock: what you own, what is charged against it and what it yields.

The three parts of the assets and liabilities statement
The three parts are real estate, other liabilities including guarantees and leasing, and other assets such as bank balances, securities and participations, closing with a declaration and space for certification by a tax advisor or auditor.

Twelve rows, one per property. Why the bank wants it in addition to the self-disclosure
The self-disclosure asks for income and outgoings — it describes the current month.
With several properties, with participations or on larger financings, the self-disclosure alone does not answer that for the bank.

How do you fill in the assets and liabilities statement for a German bank correctly?
Fill in the statement part by part: one row per property, each liability separately, each asset with evidence. Amounts are in thousands of euros (TEUR), as the column headers show.
Part A: real estate — One row per property with address and land-register entry; rent as annual net rent, target and actual separately.
For part ownership, state the total and your own share. Interest, repayment, fixed-rate period — Interest rate, repayment rate and the end of the fixed-rate period are in the loan agreement or on the bank's annual statement. Part B: liabilities — Contingent liabilities too: guarantees, rent guarantees, leasing.
Attach the contracts. Part C: other assets — Bank balances, surrender value of life insurance, securities, fund units and shareholdings — each with evidence. Closing — Date and signature; for larger financings also the confirmation of a tax adviser or auditor.
